Promoting Home Health in Assisting Living

Ohio Living Home Health & Hospice offers affordable, comprehensive programs that give adults the help they require and the confidence they need to stay at home, living the life they choose. A menu of options allows those we serve to select as much or as little assistance as they need.

Since 1980, Ohio Living Home Health & Hospice has grown from a few clients to serving more than 67,000 people throughout Ohio. Our staff establishes a strong relationship with each patient built on kindness and trust, which has allowed us to deliver services to more families in their homes.

Hospice Care When — and Where — Your Patients Need It

The Patient's Home
Hospice services can be provided in a patient’s residence, whether they live by themselves, with a spouse or significant other, or another member of their family. The Ohio Living Home Health & Hospice team provides physical, emotional and spiritual care, and supports their primary caregiver and family members as well.

Assisted Living Facility
If a patient lives in assisted living, the facility provides custodial care — such as help with activities of daily living — while the hospice team is responsible for the professional management and support related to the client’s care needs. Ohio Living Home Health & Hospice can provide hospice care in any assisted living facility within our service area.

Skilled Nursing Facility
Most skilled nursing facilities contract with hospice providers to determine up front which services will be provided by whom, and how they will communicate to make sure the patient’s needs are met 24 hours a day. Typically, the facility retains responsibility for the patient’s custodial needs, and Ohio Living Home Health & Hospice manages hospice services according to the patient’s plan of care.

Palliative Medicine and Hospice: Know the Difference 

Patients who are experiencing pain, shortness of breath, nausea, anxiety, depression, frequent hospitalizations or visits to the emergency department may benefit from palliative care, which can be provided at any stage in the illness, including during aggressive or curative treatments.

Ohio Living Palliative Medicine’s team is comprised of compassionate, experienced nurse practitioners and other specialists who work with you to provide relief for your patients who have a chronic disease and the need for pain or symptom management.

We offer your patients:

  • In-depth experience working with whole-person care that can relieve symptoms of a disease or disorder, regardless of whether it can be cured.
  •  A full range of services provided by one of our nurse practitioners, based on your patient’s needs and requests.
  • Family meetings to discuss goals of care and to make treatment choices.
  • A personalized treatment plan with goals specific to your patients’ needs, concerns, symptom management and relief from suffering.
  • Coordination of care across health care services to maximize the quality of life along the continuum of the illness.
