Assisted Living offers you the choice of apartment with the personalized care and services you need. Whether it’s providing a little extra help with daily dressing or managing medications, Ohio Living Swan Creek’s assisted living is the answer. Services are tailored to each person we serve.

Ohio Living Swan Creek is certified by the Ohio Department of Aging to provide services under the Assisted Living Waiver Program. Individuals who are enrolled or are currently enrolling in the Assisted Living Waiver Program are accepted based on availability.
