Agape Crossing at Ohio Living Swan Creek is a Certified Memory Care Center of Excellence through the Hearthstone Institute, exclusively serving residents living with dementia. As a Center of Excellence, we embrace the I’m Still Here® philosophy and approach, which treats each resident with respect and dignity, through an understanding of who they are outside of their disease.

Our team members are specially trained with knowledge and skills that help them more effectively serve residents and enhance their quality of life through their interactions, communication, and environment.

Accommodations include fully furnished private or semi-private suites and 24-hour nursing care and security.
